Pomalidomide is an oral immunomodulatory drug (IMiD) primarily used in the treatment of relapsed or refractory multiple myeloma, particularly in patients who have already received other therapies such as lenalidomide and bortezomib. Structurally related to thalidomide, pomalidomide works by modulating the immune system, inhibiting tumor growth, and blocking the formation of new blood vessels (anti-angiogenesis). Its mechanism of action involves binding to cereblon, a protein that plays a key role in targeting certain transcription factors for degradation. This leads to reduced survival of myeloma cells and enhanced immune-mediated destruction of abnormal cells. Pomalidomide is usually used in combination with dexamethasone and sometimes other agents in clinical settings.
